Self-Managed packages allow the package owner to have more control over their services, by allowing the client to pick their own care workers and support services.

Package Management

The Accounts team, manages the financial components of the package including the payment of invoices, reimbursements, and provide monthly statements of your expenses and balances.

Care Management

The Care team, create a personalised care plan & budget with you. Trilogy Care updates Care Plans frequently, whenever a customer contacts us, requiring amendments to their plan, or their health has changed. Formal changes are made yearly.

Workforce Compliance

The Compliance team, ensure worker compliance - ABN, Police check, Proof of Covid-19 infection control training & vaccination, public liability and indemnity insurance.

Care Coordination

A key feature in our self-managed PLUS package offering. The Care Coordination Program is designed to assist our clients manage components of their package that may be difficult to manage if there are no family or friends to support them.

Business Development

Our business development team, have developed a national network of health professionals, organisations and independent contractors who partner with Trilogy Care to deliver greater choice, transparency and value to our new and existing client base.

Community Managed Care

Our Process

Once onboarded with Trilogy Care, our process is simple to follow, making it more convenient for you to focus on acquiring care services, and living an independent, healthy, and supported life.

Care Plan & Budget

The client’s needs are assessed, and a care plan & budget are developed with them to provide coordinated care and services to meet their needs, preferences and goals; using their Home Care package.

Care Services

The client can procure their own workforce and suppliers to deliver their care plan. Clients can negotiate with support workers on rates, time, and services provided.

Package Management

Trilogy Care obtains the Home Care funding from the government each month and pays approved invoices or reimburses the client for care and services.
