Self-management empowers consumer to manage their care and services themselves. By doing so, consumers have control over their workforce, can find support workers themselves and negotiate rates they are comfortable paying.
If consumers self-manage your Home Care Package they can basically double the hours of care across all package levels. By partnering with Kaleidoscope Care contractors offer much better rates than most providers and Trilogy Care will manage their package for a much lower rate too.
We are completely transparent in our pricing by offering two fixed services.
15% Fully self-managed
Ideal for individuals who want to do it all themselves or with family. They will coordinate their own workforce.
26% Self-Management Plus
Ideal for individuals who want the benefits of self-managing with some help from Trilogy Care. Trilogy will do the Care Coordinating for them.
